Starting October 22, 2021, the Vaccine Passport system will be used to verify a person’s COVID-19 vaccination status before accessing certain Town facilities. This is following a direct mandate from the Government of Newfoundland and Labrador, in order to protect residents and help us live safely with COVID-19, while still allowing activities and events to take place.
Proof of Vaccination will be required at the following venues:
Conception Bay South Arena (spectators 12 and over, walking track users, and ice surface users 19 and over)
Robert French Memorial Stadium (spectators 12 and over, and ice surface users 19 and over)
Conception Bay South Recreation Complex (pool and squash court users 19 and over)
Rented Town facilities (individuals/groups renting facilities or facility spaces are required to ensure anyone entering the building for their gathering provide proof of vaccination)

For individuals with a medical exemption, until QR codes have been processed by the Government of Newfoundland Labrador, an exemption letter from a physician must be provided.
What is required to enter a facility?
Proof of vaccination (QR code from VaxPass App, paper copy, card)
1 form of photo identification (children ages 12-18 may provide a non-photo identification which displays their name and date of birth. Individuals 19 and over without a photo ID must provide 2 forms of identification, both containing their name and one that contains their date of birth. )

Visit your local public library if you do not have access to a computer or mobile device. Please have your MCP with you.
Call 1-833-951-3859 to request a paper copy. You will need to provide your MCP number for the request to be processed.

Your COVID-19 vaccination record is stored securely and recorded in the Electronic Medical Record (EMR) at public health clinics, pharmacies and physician offices. The COVID-19 vaccination record QR code contains your name, date of birth, and vaccination details. Please do not share your QR code on social media. You should only share your QR code with trusted businesses and organizations using the NLVaxVerify app.
Residents 19 years of age or older, will be required to display a photo identification and their COVID-19 vaccination record in the form of a QR code using the NLVaxPass app or using a printed copy. Residents 12 to 18 years of age will be required to display an identification with their year of birth and name, along with their COVID-19 vaccination record QR code.
The Town will use NLVaxVerify to scan the COVID-19 vaccination QR code and verify your identification. NLVaxVerify is the Government of Newfoundland and Labrador’s official app that businesses or organizations can use to verify a COVID-19 vaccination record. Once the QR code is scanned by NLVaxVerify, it will display your full name along with a ‘Green’ (COVID-19 Vaccination Status met) or ‘Red’ (COVID-19 Vaccination Status unmet). The name on your identification must match the name on your COVID-19 vaccination record. If it does not, you may be asked to show another piece of identification.
NLVaxVerify includes the Newfoundland and Labrador logo on the verification screen. You can request the business or organization to show you their device screen to verify they are using NLVaxVerify prior to presenting your QR code. NLVaxVerify does not save your personal information. QR codes are not stored or kept on any devices, or the NLVaxVerify app.
A resident who does not display their COVID-19 vaccination record with identification is not permitted to enter certain businesses or activities identified within the scope of the program.
